- Proof of Legal Name Change Document, if changing name
- Proof of WV Residence Document (2), if changing address *If you are testing for a CDL permit you must provide your DOT Card
*If you are renewing a CDL you must provide your DOT Card.
OUT OF STATE TRANSFER
- Proof of Identity Documents
- Social Security Number (Know your number or bring proof)
- Proof of Legal Name Change Document
- Proof of WV Residence Document (2)
- Driver’s License or Driving Record from previous state
*If you are transferring a CDL you must provide your DOT Card.
